Who Can Refer

We accept referrals from physicians, therapists, school professionals, and other qualified providers involved in a child's care.

Collaboration and Care Coordination

We believe that collaboration is essential to providing effective, meaningful care. With appropriate consent, we work closely with families, educators, therapists, physicians, and other professionals involved in a child's care to ensure that recommendations are coordinated and aligned across settings.

Our goal is to support a shared understanding of each child's needs and to promote consistency between home, school, and healthcare environments.

Provider Referral Process

We appreciate your trust in referring patients to our practice. This portal is designed to support providers who wish to refer children for neuropsychological evaluation or who are seeking consultation on neurodevelopmental concerns. We typically respond to referrals and consultation requests within 72 hours.

Step 1: Identify Referral Need

Providers may refer children who present with learning, behavioral, emotional, developmental, or neurodevelopmental concerns that may benefit from a comprehensive neuropsychological evaluation.

Step 2: Complete the Provider Referral Form

Referring providers may submit a referral using our Provider Referral Form. The form allows providers to share relevant background information and referral questions to help determine whether a neuropsychological evaluation or consultation is appropriate.

Please do not include personal health information (PHI) in the referral form. If additional records are needed, we will coordinate secure record sharing with the family after consent is obtained.

Step 3: After Submission

After a referral is submitted, our team reviews the information to determine appropriateness and next steps. When indicated, we contact the family directly. Referring providers may be contacted for clarification or consultation as needed.

Step 4: Provider Consultation (When Indicated)

When additional clarification is needed or when an alternative approach may be more appropriate, we may contact the referring provider to discuss the referral and determine next steps. These may include a comprehensive evaluation, consultation, or alternative recommendations.

Step 5: Family Contact (When Appropriate)

If an evaluation or consultation is recommended, we contact the child's parent or guardian to review the evaluation process, obtain informed consent, and coordinate scheduling.

Step 6: Evaluation Process

When indicated, the evaluation typically includes clinical interviews, standardized testing, behavioral and emotional assessment, and review of relevant records, as appropriate, to understand the child's cognitive, emotional, behavioral, and social functioning.

Step 7: Feedback and Reporting

Families receive a comprehensive written report summarizing findings and recommendations. A feedback session is scheduled to review results, answer questions, and discuss next steps. With appropriate consent, findings may also be shared with referring providers and school teams.

Step 8: Follow-Up and Collaboration

We remain available for ongoing collaboration with families and providers to support treatment planning, educational decision-making, and coordination of care as needed.
