Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) may be used when permitted by your plan. Families are encouraged to confirm eligibility with their plan administrator.
A valid credit or debit card is required to be placed on file at the time of scheduling.
A non-refundable deposit equal to 25% of the total evaluation fee is required before evaluation appointment dates will be scheduled. Appointment dates are confirmed and reserved only after the deposit payment has been successfully processed and cleared.
The remaining balance is due on the day of the parent/child intake interview, which marks the beginning of evaluation services. The remaining balance must be successfully processed and cleared in order for the intake interview and subsequent evaluation services to proceed as scheduled.
If required payments have not cleared by the day of the intake interview, Pediatric Neuropsychology Associates PLLC reserves the right to cancel the intake appointment and any subsequent evaluation services.
